| 1. Registration. If your first visit with
us is coming up, print and fill out. Please take a moment to consider everyone
you want to have authority on your account and enter their names. Register all

will be leaving your companion with us for the day for examinations, surgery,
medical progress exams, etc. Give us the details of what you have observed and
what services or products you need. |
